The Local Research and Ethics Committees of the donating NHS Trusts require Medical Solutions to supply a completed 'Biomaterial Products Request Form' to facilitate review of the use of donated surplus tissue. If you have not already completed a Request Form then please download and submit one now as requests for supply cannot be accommodated without a fully completed request form. |
